Program Picks
Everything you need to know about choosing your programs at QVSS!
Below, you will find the summer program package to review and complete. Please return your pick sheet to QVSS staff by April 26th, 2025, to secure your spot in your chosen activities! All spots are given on a first come-first served basis.
- All programs begin at 9:00 am and end at 3:00 pm. Please arrive on time for your program. If you arrive late, you may have to make alternative transportation arrangements to get to your activity or even to return home.
- Please consider transportation pick-up when choosing your programs since some occur in the building and others in the community.
- All QVSS programs are eligible for passport funding.

Download the program package below to look over and fill out. Please return your pick sheet to any QVSS staff member by April 26, 2025, to secure your spot in your chosen activities.
When completing the program request form, we ask that you keep the following in mind:
- Some activities require having belongings with you at the beginning of your program (ex., running shoes, swimsuit and towel, coffee money, etc.). Please sign up for programs that you can be prepared for!
- While we try our very best to make all programs accessible for all, there may be programs that present challenges for certain individuals. If you or your loved one have questions about program suitability please feel free to contact Shannon.
- You can only choose the number of days you have allotted already. If you wish to increase your days with us or need to switch days, please call me, and we can work together on this.
Instructions:
- Please make sure that you put your name on the survey!
- Please choose the program you want to participate in in order of preference.
- Indicate by circling or shading your 1st, 2nd, 3rd, etc., choices underneath the program name and in a coinciding colour.
