Everything you need to know about choosing your programs at QVSS!
Below is the summer program package for your review and completion. Please return your pick sheet to QVSS staff by May 1, 2026, to secure your spot in your chosen activities! We recognize that some of our more popular programs fill quickly. We make every effort to ensure that everyone has an opportunity to participate and enjoy these activities.
- All programs begin at 9:00 am and end at 3:00 pm. Please arrive on time for your program. If you arrive late, you may have to make alternative transportation arrangements to get to your activity or even to return home.
- Please consider transportation pick-up when choosing your programs since some occur in the building and others in the community.
- All QVSS programs are eligible for passport funding.

When completing the program request form, we ask that you keep the following in mind:
- Some activities require you to have belongings with you at the beginning of your program (e.g., running shoes, swimsuit and towel, coffee money, etc.). Please sign up for programs that you can be prepared for!
- While we try our very best to make all programs accessible to all, some may present challenges for certain individuals. If you or your loved one have questions about program suitability please feel free to contact Shannon.
- You can only choose the number of days you have already allotted. If you wish to increase your days with us or need to switch days, please call me, and we can work together on this.
Instructions:
- Please make sure that you put your name on the survey!
- Please choose the program you want to participate in in order of preference.
- Indicate by circling or shading your 1st, 2nd, 3rd, etc., choices underneath the program name and in a coinciding colour.
